Curious about dance? Our free discovery classes invite your dancer to step into the studio and experience the excitement of dance, all with no commitment. It's a fun, no-pressure class where your child can explore movement, rhythm, and creativity in a welcoming, supportive environment. Whether they’re new to dance or just looking to try something different, this offers a great way to see if dance is the perfect fit—simply come, enjoy, and discover the magic!
Liability
Waiver
As legal guardian of the above named student(s), I allow participation in the activities provided by Center Stage Dance, including but not limited to all aspects of dance training and/or competition, I am fully aware that any activity involving motion, height, or athletic activity creates the possibility of serious injury. I hereby release Center Stage Dance, including its officers, shareholders, agents, and employees, from any liability to the above named student(s), or the person(s) claiming through him/her, arising from injury to the person or property of the above named student(s) occurring on the premises of Center Stage Dance, including any event sponsored or sanctioned by Center Stage Dance, and/or travel to and from such activities.
This release includes but is not limited to any claims of negligence, dangerous condition, latent defect, premises liability, code violation, negligent security, failure to warn, vicarious liability, negligent hiring, negligent supervision, negligent maintenance, or improper/dangerous equipment; it is intended to be as broad as permissible under Nebraska law.
I am fully aware of the nature of the activities provided and the possibility of injuries arising from such activities. I further agree to hold harmless, indemnify and defend Center Stage Dance, including its officers, and employees from any loss, liability, damage, or cost incurred by them due to the above named student(s) on the premises or during any event sponsored or sanctioned by Center Stage Dance. I also attest that all information given is factual.
I certify that the student(s) is/are in good health and may participate in activities at Center Stage Dance. In case of an emergency requiring medical treatment, the undersigned hereby authorizes Center Stage Dance, to take the above named student(s) to a qualified medical or hospital facility for care and treatment.
Appearance Release
This Release is made to allow Center Stage Dance and its subsidiaries, affiliated companies, successors, assigns and licensees to film, photograph and otherwise record or use the below named student(s) name, voice, likeness, appearance, actions, voice, conversations and characteristics/mannerisms (collectively, the Appearance) and use such Appearance as set forth herein.
I acknowledge and agree that Center Stage Dance shall be the exclusive owner of all copyright and other rights in and to such Appearance, as ordered, created and/or commissioned by Center Stage Dance, and I hereby irrevocably and unconditionally grant to Center Stage Dance the right (but not the obligation) to utilize the Appearance, in connection with any and all purposes of Center Stage Dance, including but not limited to:
a. Marketing, advertising and promotion of products and services;
b. Educational, instructional and informational content;
c. Appearances at conferences, competitions, industry events, and other meetings; and
d. Any and all other content created or distributed by or for Center Stage Dance.
I agree that any interviews, testimonials or other statements made in the Appearance relating to Center Stage Dance, or in writing, reflect my current honest and truthful opinions, findings or experience. Without limiting the generality of the foregoing, Center Stage Dance shall have the right to add to, subtract from, create derivative works based on, change, revise, rearrange or otherwise edit the Appearance. Center Stage Dance shall have no obligation to use the Appearance, but may do so with or without any payment to myself or any other person or entity.
To the maximum extent permitted by law, I hereby release Center Stage Dance, its successors, assignees and licensees and each of their respective parents, subsidiaries and affiliated companies, from any claim of any kind or nature whatsoever arising from the use of the Appearance including, without limitation, any and all claims, demands, or liabilities for invasion of privacy, infringement of my right of publicity, defamation (including libel and slander) and any other personal and/or property rights. I intend and agree that this Release shall be effective as a full and final accord and satisfaction and general release. All controversies or questions with respect to this Release shall be determined in accordance with Nebraska law. This Release contains the full and complete understanding between us with respect to the Appearance and cannot be modified except by a writing signed by Center Stage Dance.
